Is this truck worth it?

The knock in the motor is probably a rod. You'll have to either replace, or rebuild the motor. A rebuilt short block will run 1500 and up, depending on the shop and work done. And 200k miles, the rest of the drive train will need work soon if nothing has been done yet. With that many miles on it, and having rust, I'd suggest keep looking.
